A client centered, nursing based assessment evaluates a client's ability to participate in their care and determine what assistance is needed in order for the client to achieve their maximum psycho, social, physical and spiritual potential.
- Suzanne's strong clinical emergency nursing skills and her extensive knowledge of the federal (CMS) and Washington State long term care regulations result in her performing a thorough & comprehensive assessment.
- Through observation, interview and record review, Suzanne visits the client in their home to capture an accurate and personalized assessment.
Once an assessment is completed, an individualized care plan can be developed to assist caregivers with care. A care plan also allows for the identification of high risk areas (such as skin break down, falls, & difficulty swallowing) so caregivers can implement identified measures in an attempt to avoid any negative outcome to the resident.
